Flagler County residents now have a new option for medical care close to home. AdventHealth has opened a combined urgent care and primary care facility on State Road 100 in Palm Coast, bringing two types of services together under one roof for the first time in the county.

The new location houses both a Centra Care urgent care clinic and a Primary Care+ office. It is the second AdventHealth urgent care site in Flagler County — joining an existing Centra Care near Palm Coast Parkway — and the first Primary Care+ location in the county.
“Having access to care close to home makes a real difference for families,” said Denyse Bales-Chubb, president and CEO of AdventHealth Flagler and St. Johns Counties. “This new location on State Road 100 helps remove barriers, whether someone needs care today or is looking for a trusted primary care provider who can support their health over time.”
What Each Location Offers
The Primary Care+ office serves patients ages five and older and provides annual wellness visits, preventive care, immunizations, school and sports physicals, sick visits, lab services, and chronic disease management. The office also offers same-day and next-day appointments, evening hours, video visits, and access through the AdventHealth app.
The Centra Care clinic provides walk-in treatment for non-life-threatening illnesses and injuries such as sprains, minor infections, fevers, and cuts. No appointment is needed.
AdventHealth leaders said combining both services in one location is designed to help residents get care earlier and reduce unnecessary emergency room visits.
“This location gives the community a nearby place to turn when a question comes up or an issue can’t wait,” said Robert Paswaters, chief operating officer of AdventHealth Retail Services. “If you wake up sick, Centra Care is here.”
“And when you’re focused on staying healthy, Primary Care+ is designed to be an easy first stop,” said Jessica Baird-Wertman, CEO of AdventHealth Primary Care. “Board-certified providers offer convenient hours and are connected to the full resources of our system when more longitudinal care is needed.”
